Scheduling
To schedule, confirm, or change an appointment, please call 317-886-7312. If I am in an appointment, your call will transfer to my voicemail system. Please feel free to leave a confidential message, and I will return your call as soon as I can. Please note that it may take 24 to 72 hours for me to get back to you.
In addition to contacting me via telephone, I can be reached via email at jcacciola@uncoveringstrengths.hush.com. Please note that if you choose to send an email directly to me using a standard email service, you understand the risks of sending Protected Health Information (PHI) through email and accept these risks.
I complete a brief telephone call with individuals to better understand why they are seeking an evaluation. If for any reason I do not think I am the best fit for completion of an evaluation for you/your loved one, I will let you know this during the call and provide a list of providers who are better equipped to answer your referral question(s).
Pricing Information
The typical range for completion of a comprehensive evaluation is between $1900 to $2500**. I provide this range, and ask individuals/families to consider paying an amount that is financially accessible and that is commensurate with their level of financial capacity/privilege.
**As of January 1, 2025, the range for completion of a comprehensive evaluation will be between $2,000 to $2,600
Paying the full fee if it is financially accessible helps those who have less financial capacity/privilege access evaluation services with less financial strain and is in alignment with efforts to promote equitable access to evaluation services. It allows me to run a sustainable business that promotes the highest possible quality evaluation services for all children, youth, and adults, as well as supporting my wellness as a mental health professional to ensure I show up from a space of curiosity, authenticity, and active engagement. It additionally allows me to offer reduced fee evaluations and partner with other agencies to provide some pro bono evaluations each year.
The range for completion of a comprehensive evaluation was determined with the understanding that many individuals/families will pay the base level fee, and I am aware that the overall cost may impact whether individuals can pay the full cost/full fee for completion of evaluation services. If you/your loved one is in need of a reduced fee evaluation, please let me know. If I am not able to accommodate this need, I will do my best to identify providers who may be able to do so.
The evaluation process includes time spent in direct contact with individuals and/or caregivers via clinical interviews and caregiver interviews for children and adolescents, face-to-face testing, scoring and interpreting assessment data, reviewing records, completing collateral calls with providers or support persons (if/as appropriate and with a release of information), writing the evaluation report, and providing feedback.
Full Fee for 2024: $2300 to $2500 (Full Fee for 2025 will be $2400 to $2600)**
At this fee, you are supporting and uplifting others, and helping me run a sustainable business that promotes the highest possible quality evaluation services for all individuals. This also allows me to offer reduced fee evaluations and partner with other agencies to provide some pro bono evaluations each year in an effort to promote equitable access to evaluation services.
*I commit to completing reduced fee or pro bono evaluations that are equal to at least 10% of business profits for the year. The total percentage may vary year to year based on the ebb and flow of business profits and costs associated with maintaining the practice.
Middle Fee for 2024: $2100 to $2200 (Middle Fee for 2025 will be $2200 to $2300)**
At this level, you are helping me run a sustainable business that promotes the highest possible quality assessment services for all individuals, including you/your loved one.
Base Fee: $1900 to $2000 (Base Fee for 2025 will be $2000 to $2100)**
At this level, you are paying for highest possible quality assessment services for you/your loved one. My hope is that you will find ways to support others in your community in whatever way you can, financially or otherwise, if and when you have the capacity to do so.
**I invite individuals/families to choose the total fee they pay within each associated fee level based on their current financial accessibility/capacity.
Insurance Information
I do not accept or negotiate fees with insurance at this time. However, I can provide you with a superbill upon request if you would like to submit it for possible Out of Network Coverage (OON) reimbursement. I cannot guarantee OON reimbursement. Please check with your insurance carrier to determine whether you have OON benefits, and what OON benefit policies are. Individuals are required to submit, and manage correspondence regarding, the superbill with their insurance carrier.
Payment
I understand that fees for completion of an evaluation are large. Some individuals choose to pay for the evaluation in full at the time of their/their loved one’s evaluation appointment. More typically, individuals pay half of the evaluation fee at the time of the appointment, and pay the other half of the fee as the evaluation report is being finalized. Due to some individual’s circumstances, they may need to create another type of payment plan. As such, please let me know if collaboratively creating an alternative payment plan would be helpful. With payment plans, a payment installment will be due each month until the evaluation fee has been paid in full.
In families where caregivers do not share the same household, payment is the responsibility of the caregiver who attends the evaluation appointment with their child.
Payments in the form of cash, check, or money order are accepted. I do not have a credit card processor, and as such, I cannot take payment via credit card, HSA cards, or Care Credit. Some individuals also choose to make payments via Zelle.
Cancellation Policy
I understand that illness and unforeseen events or emergencies are inevitable. If you need to cancel your appointment, please provide notice 24 hours in advance of your/your loved one’s scheduled appointment. Cancellations received with less than 24 hours’ notice, as well as No Showing scheduled appointments, may incur a cancellation fee, which is separate from the total payment to complete the evaluation. Each individual’s circumstance/situation will be discussed to explore next steps.
Good Faith Estimate
Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
